UC §59-10-1310

You may voluntarily contribute $1 or more to provide funding for exempt organizations and city or county animal shelters where licensed veterinarians provide spay or neuter services for dogs and cats owned by persons having low-incomes.

Any contribution you make will add to the tax due on your return or reduce your refund. Once made, you cannot change the contribution on an amended return. 

Write the code (09) and the amount of your contribution on your Utah TC-40, page 2, lines 28a through 28d. Total all contributions on line 28.
