You may voluntarily contribute to provide funds to school districts and nonprofit school district foundations established to assist in the development and implementation of authorized programs to promote educational excellence and to assist in the accomplishment of other education-related objectives. If the school district does not have a nonprofit foundation, the contribution goes to the school district.
Any contribution you make will add to the tax due on your return or reduce your refund. Once made, you cannot change the contribution on an amended return.
Write the code (05) and the amount of your contribution on your Utah TC-40, page 3, Part 4. Total all contributions on line 28.
Indicate to which school district you would like your contribution donated by entering the corresponding code in the "SCH DIST CODE" space to the right of the amount on page 3, Part 4, of your TC-40.
