You may voluntarily contribute $1 or more to provide funds for services and programs statewide to help Utahns become self-sufficient. Programs funded include emergency housing, placement in meaningful employment or occupational training activities, and special services to meet the unique needs of the homeless who have families with children, or who are mentally ill, disabled, or suffer from other serious challenges to employment and self-sufficiency.

Any contribution you make will add to the tax due on your return or reduce your refund. Once made, you cannot change the contribution on an amended return. 

Write the code (02) and the amount of your contribution on your Utah TC-40, page 2, lines 28a through 28d. Total all contributions on line 28.
