Utah Code §59-10-1315

You may voluntarily contribute $1 or more to provide funds for the purchase of personal protective body covering, whether worn alone or as a complement to another product or garment, and is intended to protect a police service canine (dog) against gunfire. 

Any contribution you make will add to the tax due on your return or reduce your refund. Once made, you cannot change the contribution on an amended return. 

Write the code (11) and the amount of your contribution on your Utah TC-40, page 2, lines 28a through 28d. Total all contributions on line 28.
